Western Bulldogs Terms and Conditions
please read and agree to the following terms and conditions:
1)This Event is run by the Western Bulldogs
2) Once this form is signed, the Western Bulldogs Football Club/Western Bulldogs Community Foundation, and third party program partners have permission to use any statements I make or photos of me related to the WBCF event in any form of media (or otherwise) without paying me to use them.
3) The Western Bulldogs Football Club/Western Bulldogs Community Foundation and partners has the right to:
a. cancel or suspend the WBCF event at any time (with less than 2 weeks notice if restricted by government)
b. refuse my attendance in the WBFC and WBCF event with valid reason
c. change these terms and conditions and;
d. put a limit on the maximum number of participants at venue
a. any personal illness/injury that could result in loss of function and reduced work ability
4) I am responsible for the costs of treatment and transportation deemed necessary from emergency/medical services.
5) I am responsible for keeping hydrated and bringing with me any medications needed to maintain my safety during the Program;
6) I am responsible for keeping my personal and medical details up to date and telling my emergency contact that they are listed as the first point of call in an emergency
7) I agree I will not attend the WBCF event if I feel unwell or have any symptoms of Covid-19.
8) I acknowledge the Western Bulldogs Community Foundation and partners will follow safety guidelines from Vic Gov and DHS to keep me safe. I understand that I might be at risk of contracting Covid-19 by attending face to face sessions with other community members. I
How will my information be used?
The Western Bulldogs and Western Bulldogs Community Foundation and partners respects the privacy of all your information. Any personal information collected by us will be used for the following purposes: to help you stay safe at sessions by letting staff know of any health concerns putting you at risk, program administration, to contact you during the program or in the future, research and program evaluation.
By agreeing to these terms and conditions, you agree that de-identified information about your interaction with the WBCF event may be used for research purposes. This information will not have your name on it and your identity will remain anonymous.
Who has access to my information?
Relevant Western Bulldogs and partners staff and program coordinators at the session are the only people who will have access to your information. It may be shared with medical professionals in an event that they require access to this information for medical treatment.
How will my information be stored?
All information will be securely and confidentially stored on password encrypted systems used by the Western Bulldogs Community Foundation and partners. The Western Bulldogs Community Foundation and partners will take reasonable steps to ensure all partners to whom it transfers your personal information adhere to the Privacy Act 1988 (Cth) and Australian Privacy Principles.
